IBMS Center of Healthcare Excellence(Hospitals, Clinics, Specialty Centers)Application Fee is $2,500.00.

By signing/notarizing the IBMS Center of Healthcare Excellence (Hospitals, Clinics, Specialty Centers) Application, the applicant agrees to having provided accurate information and to the IBMS Non-Refundable Fee Policy.

The applicant recognizes and agrees no monetary damages, whether compensatory, consequential, collateral, punitive or otherwise, would be entitled from IBMS or any of IBMS Representatives, Associates, Quality Reviewers, or Affiliates, as a result of any controversy/claim with IBMS. Claim would be amenable to arbitration with waiver of rights to initiate legal action.

If the IBMS Center of Healthcare Excellence (Hospitals, Clinics, Specialty Centers)Application is not approved/accepted, 50% IBMS Application Fee less bank fees/charges shall be refunded.

IBMS Center of Healthcare Excellence (Hospitals, Clinics, Specialty Centers) Application & Certification Fees must be paid in full prior to IBMS Quality Certifying Review.

Within 1 month of receipt of the IBMS Center of Healthcare Excellence (Hospitals, Clinics, Specialty Centers) Application/Fee, the IBMS CHE Applicant will be advised whether the IBMS CHE Application is approved/accepted.

Upon IBMS Board Approval/Acceptance, the CHE contact will be advised/invoiced the IBMS Center of Healthcare Excellence(Hospitals, Clinics, Specialty Centers) VIRTUAL ONLINE/ONSITE Standards Checklist Fee.

IBMS STANDARDS CHECKLIST

• Upon receipt of the IBMS Center of Healthcare Excellence (Hospitals, Clinics, Specialty Centers) VIRTUAL ONLINE Standards Checklist Fee, IBMS will send the IBMS CHE Standards Checklist - Short/Long Form* and assign an IBMS ONLINE Quality Certifying Reviewer to review/certify the IBMS CHE Standards Checklist - Short/Long Form* with the identified CHE Contact.
• Upon receipt of the IBMS Center of Healthcare Excellence (Hospitals, Clinics, Specialty Centers) ONSITE Standards Checklist Fee, IBMS will send the IBMS CHE Standards Checklist - Short/Long Form* and assign an IBMS ONSITE Quality Certifying Reviewer who will schedule the ONSITE visit to review/certify the IBMS CHE Standards Checklist - Short/Long Form* with the identified CHE Contact.

IBMS QUALITY MULTIDISCIPLINARY ASSESSMENT SURVEY

• Upon receipt of the IBMS Center of Healthcare Excellence (Hospitals, Clinics, Specialty Centers) VIRTUAL ONLINE Quality Multidisciplinary Assessment Survey Fee , IBMS will assign anIBMS ONLINE Quality Certifying Reviewer to review/certify the VIRTUAL ONLINE Quality Multidisciplinary Assessment Survey with the identified CHE Contact.
• Upon receipt of the IBMS Center of Healthcare Excellence (Hospitals, Clinics, Specialty Centers) ONSITE Quality Multidisciplinary Assessment Survey Fee, IBMS will assign an IBMS ONSITE Quality Certifying Reviewer who will schedule the ONSITE visit to review/certify the VIRTUAL ONLINE Quality Multidisciplinary Assessment Survey with the identified CHE Contact

IBMS Final Quality Assessment Summary Evaluation Report Review/Summary/Waivers/Recommendations/Certification Approval

• Upon receipt of the IBMS Center of Healthcare Excellence (Hospitals, Clinics, Specialty Centers) VIRTUAL ONLINE Application, IBMS FINAL QUALITY ASSESSMENT SUMMARY EVALUATION REPORT Review/Summary/Waivers/Recommendations/Certification Approval Approval Fee, IBMS will assign an IBMS ONLINE Quality Certifying Reviewer to complete the IBMS Final Review/Summary/Waivers/Recommendations/Certification Approval Process with the identified CHE Contact.
• Upon receipt of the IBMS Center of Healthcare Excellence (Hospitals, Clinics, Specialty Centers) ONSITE IBMS Final Review/Summary/Waivers/Recommendations/Certification Approval Fee , IBMS will assign anIBMS ONLINE Quality Certifying Reviewer to complete the IBMS FINAL QUALITY ASSESSMENT SUMMARY EVALUATION REPORT Review/Summary/Waivers/Recommendations/Certification Approval /Certification Approval Process with the identified CHE Contact.

Subsequent to the IBMS FINAL QUALITY ASSESSMENT SUMMARY EVALUATION REPORT / Review/Summary/Waivers/Recommendations/Certification Approval an Approval Process , IBMS Center of Healthcare Excellence Certification shall be issued.
